The CDCPOpens external website in a new window will help ease financial barriers for Canadian residents that have an adjusted family net income of less than $90,000 and don’t have access to employer sponsored or private dental coverage.

The CDCP will be rolled out using a phased approach (starting with seniors, persons with disabilities and children under 18).

Eligible Canadian residents will be able to apply to the CDCPOpens external website in a new window through the Government of Canada.

Once deemed eligible, Canadian residents’ information will be shared by Service Canada to Sun Life for CDCP enrolment. Once enrolled, individuals that are covered under the CDCP will receive a welcome package with coverage details (including a member card and their coverage start date). 

The coverage start date is determined based on when an individual’s application was received and when an individual was enrolled in the CDCP.

CDCP clients can see any oral health provider they choose for their care, as long as the provider agrees to direct bill Sun Life for services provided under the plan. To find a provider near you, use the CDCP Provider Search tool, call your local provider, or call us at 1-888-888-8110. You should not pay your provider for the full cost of your treatment and submit a request to Sun Life for reimbursement, as you will not be reimbursed.

Members should bring their member card and valid identification to their oral health care appointments. This will help participating providers confirm CDCP member eligibility with Sun Life quickly.

Only members can use their member card. Family members that are covered under the CDCP will get their own member card once their enrolment is approved.

Sun Life will not reimburse CDCP Clients. Claims for services covered under the CDCP must be submitted by a participating CDCP oral health provider directly to Sun Life. The CDCP will pay for any eligible CDCP services directly to the participating provider.

For services that aren’t covered by the CDCP, members will have to pay for those services directly to their oral health provider, if applicable.

CDCP Provider Search works by selecting the centre point of your location (address, city and/or postal code) and populating providers within a 50 km radius.

You can find your benefit effective date, as well as the CDCP contribution percentage of eligible CDCP fees, on the same document as your member card. The date is not included on your member card.

Your coverage must be in effect at the time of your appointment.

• Providers must bill eligible CDCP expenses to Sun Life directly. 
• Never pay the full amount of the services in advance, as Sun Life will only reimburse your provider. Sun Life will not reimburse you for services under the CDCP.
• The CDCP may not pay the full cost of treatment. You are only responsible for paying any potential outstanding amount to your provider. 

CDCP members may be required to pay a portion of the cost when visiting an oral health provider. 

The CDCP will reimburse a percentage of the cost, based on established CDCP fees and your adjusted family net income. You may have to pay additional charges directly to the oral health provider, if:

• your adjusted family net income is between $70,000 and $89,999
• the cost of your oral health care services are more than the established CDCP fees, or
• you and your oral health care provider agree to services that the CDCP doesn’t cover 

The CDCP will only pay providers for oral health care services covered within the plan and in accordance with the CDCP established fees. 

You can only coordinate benefits with another federal, provincial, or territorial social dental program and not with any privately funded or employer funded dental benefits program.
